REFERRAL CONTEST RULES

1. Prize Description: 2 Bass Pro Zero Gravity chairs and bronze steel fire pit drawing…ODDS OF WINNING IN THIS PARTICULAR DRAWING DEPEND ON THE NUMBER OF ELIGIBLE ENTIRES RECEIVED FOR THE DRAWING. VOID WHERE PROHIBITED
2. Eligibility: Drawing is open to legal residents of the United States of America whom are patients of Wilkinson Dental and have referred at least one friend or family member; Wilkinson Dental office staff and the immediate family and household members of each are not eligible.
3. Sponsor: Wilkinson Dental, 1902 E. Battlefield, Ste A, Springfield, MO 65804
4. Agreement to Official Rules: By participating in this drawing, each entrant fully and unconditionally agrees to and accepts these Official Rules and the decisions of Sponsor, which are final and binding. Entrants also fully agree to release all media associated with the drawing to Wilkinson Dental, which has the right to publish the media on any and all media mediums.
5. Timing: September 1, 2015 thru November 30, 2015 at close of business day.
6. How to Enter: Entries will automatically be entered by the staff at Wilkinson Dental as follows. 1) One entry for each person that is named as referring a new patient to the office that completes a new patient appointment.Only one person may be entered for referring each patient.
7. Drawings and Prize Notifications: The Fall New Patient Referral Prize drawing will be conducted in accordance with these Official Rules on or about November 30, 2015 at 5:00 pm. All drawings will be conducted by Sponsor. Sponsor’s decision as to the administration of the drawing and the selection of the winner are final. Only one winner will be selected. The winner will be sent notification of winning at the email address given at the time of entry. An attempt will also be made to contact the winner via telephone if a number is available. Winner will be required to respond as required in the notification of winning within thirty (30) days. If winner does not respond within thirty (30) days as required, the Sponsor will select a new winner and the original winner will be deemed to have forfeited the prize.
