Get entered or nominate someone you know!:

1. Starting June 01 we will be accepting video submissions via our Facebook page. Tell us in 3 minutes or less why you or a friend deserve to win. Videos will need to be submitted by messaging our Wilkinson Dental Facebook page or emailing [email protected], and attaching the video. Don’t forget to get creative!
2. Video submissions will become public on our Facebook page as soon as they are received, and the voting will begin. It is up to the community to decide who wins.
3. The video that gets the most likes on Facebook by Friday, August 28 at 5:00 p.m. will be the winner!

Official Rules and Details:

OVERVIEW:
Those interested (Entrants) will have the opportunity to submit a video entry for Wilkinson Dental’s (Sponsor) $10K Smile Giveaway anytime between June 1, 2015 and August 10, 2015. Entrants must submit a video telling why they or someone they know deserves the smile makeover. If the video includes a person other than the Entrant, the Entrant must have permission from that person to submit the video. Entrant must be able to obtain a signed release from the owner upon request of the Sponsor. Submissions should be sent via our Facebook messenger or by emailing [email protected]. Videos will then become public on our Facebook wall at the discretion of any Wilkinson Dental staff. Videos deemed inappropriate will not be posted and the entrant will be notified and disqualified from the giveaway. Entrants must submit their video by August 10th at 5:00 p.m. to be considered for Wilkinson’s Dental $10K Smile Giveaway. Videos submitted late which are considered between August 10th at 5:00 p.m. to August 11that 12:00 a.m. will be entered in a different giveaway, along with the rest of the submissions, for Dr. Wilkinson’s Smile Giveaway. The contest will end on August 28th at 5:00 p.m. The video with the most Facebook “likes” that was submitted on-time for the deadline will receive Wilkinson Dental’s $10K Smile Giveaway. The video with the most likes, excluding the winner of Wilkinson Dental’s $10K Smile Giveaway, that was submitted either on-time or late for the deadline will receive Dr. Wilkinson’s Smile Makeover. The two winners will have 30 days to claim their giveaway by calling Wilkinson Dental and confirming they accept the terms and conditions of the giveaway. If the winner does not call Wilkinson Dental within 30 days, Wilkinson Dental reserves the right to select the runner-up for that giveaway or to not proceed forward with the giveaway.

OTHER:
Wilkinson Dental’s $10,000 Smile Makeover and Dr. Wilkinson’s Smile Makeover is an in-house giveaway and is not cash value. Dr. Wilkinson’s Smile Giveaway value will be determined at the will and discretion of Dr. Trent Wilkinson at the consultation appointment for the winner. At any time, Dr. Wilkinson reserves the right to cancel Wilkinson Dental’s $10,000 Smile Makeover and Dr. Wilkinson’s Smile Makeover for unusual and unforeseen circumstances. Entrants under the age of 18 must have a parent or legal guardian present for the smile makeover and sign consent. A contestant may encourage family and friends to vote for his or her Entry, but a contestant may not attempt to induce others to vote for his or her entry through the use of any incentive, sweepstakes, or other promotion. Any use of automatic voting methods by any party will void all such votes and will result in the contestant’s entry and question being declared ineligible and disqualified from the contest. Any attempt by a contestant and/or his/her family, friends or community to vote more than one vote per day using “boot” technology and/or any other fraudulent mechanism as determined by sponsor at its sole discretion, shall give sponsor the right to disqualify the Contestant in Sponsor’s reasonable discretion.

All entrants must be able to provide their own transportation to Wilkinson Dental, 1902 E. Battlefield Rd., Springfield, MO 65804. Wilkinson Dental is not responsible for the winner’s transportation to our office location. All winners must sign a media release form giving Wilkinson Dental the right to use any photos, videos, or other media submitted for the contest and taken during the smile makeover processes. Entrants agree that the Sponsor has the sole right to decide all matters and disputes arising from this contest and that all decisions of Sponsor our final and binding.

LIMITATIONS OF LIABILITY AND RELEASE
No liability or responsibility is assume by Sponsor or Contest Parties resulting from any user’s participation in or attempt to participate in the Contest or ability or inability to upload or download any information in connection with participating in the Contest. No responsibility or liability is assumed by the Sponsor or Contest Parties for technical problems or technical malfunction arising in connection with any of the following occurrences which may affect the operation of the Contest: hardware or software errors; faulty computer; telephone, cable, satellite, network electronic, wireless or Internet conductivity or other online communication problems; errors or limitations of any service Internet service providers, servers, host or providers; garbled jumbled or faulty data transmissions; failure of any email transmissions to be sent or received; lost, late, delayed or intercepted email transmissions; inaccessibility of the website in whole or in part for any reason traffic congestion on the Internet or the website; unauthorized human or nonhuman intervention and the operation of the Contest, including without limitation, unauthorized tampering, hacking, theft, virus, bugs, worms ; or destruction of any aspect of the Contest, or loss, miscount, miss direction, inaccessibility or unavailability of an email account use in connection with the Contest. Sponsor and Contest Parties are not responsible for any typographical error’s in the announcement of prizes or these Official Rules, or any inaccurate or incorrect data contained on the website. Use of the website is at users own risk. Sponsor and the Contest Parties are not responsible for any personal injury or property damage or losses of any kind which may be sustained to user’s or any other person’s computer equipment resulting from participation in the Contest, use of the website or the download of any information from the website. By participating in the Contest the entrant releases sponsor and contest parties from any and all claims, damages or liabilities arising from or relating to such Entrant’s participation in the contest. By accepting a prize in the Contest, winners agree that the Sponsor and contest party shall not be liable for any loss or injury from the resulting from participating in the Contest, acceptance or use of any prize. Sponsor and Contest parties are not liable in the event that any portion of the Contest or scheduled commercial broadcast is canceled due to weather, fire, strike, acts of war or terrorism, or any other condition been on their control (a “Force Majeure”)event.
